Your privacy is extremely important to us. Our HIPAA policy explains how your medical information may be used and disclosed, and how you can access your information.

This document explains the nature of chiropractic care, potential risks, and your rights as a patient before beginning treatment.

This agreement outlines how disputes related to care or services may be resolved.

Appointment Cancellation Policy

Please review the following policies before your appointment.

We understand that unexpected situations may arise and you may need to cancel or reschedule your appointment. If this occurs, we respectfully ask that appointments be canceled at least 24 hours in advance.

When a patient does not attend a scheduled appointment, another patient loses the opportunity to be seen.

If we do not receive notice within 24 hours, a $10 fee will be assessed for the first missed appointment. Additional late cancellations or missed appointments may result in a $35 fee.

The cancellation fee will be waived if the appointment is canceled and rescheduled for the same day.
